Biometric Screenings and the Biometric Data Reporting form:
A biometric screening is a clinical screening that’s done to measure certain physical characteristics. It can be used to assess your:

• height
• weight
• body mass index (BMI)
• blood pressure
• blood cholesterol
• blood sugar

The goal of a biometric screening is to give a snapshot of your health and alert you to any changes in your health status.

As part of the Wellness Works! Program, employees (voluntary participation) can qualify every calendar year to receive 4 percent savings on their healthcare premiums by completing BOTH a Biometric Screening and an Online Health Risk Assessment (HRA).  In order to attend a City sponsored  Biometric Screening you must register and bring your medical card to your scheduled screening.  Biometric screenings are not a replacement for regular medical examinations or wellness visits with a health care provider. They are also not a mechanism for diagnosing disease.

Select the location you would like to attend and email the site coordinator listed on the form.

​Biometric Data Reporting Form:
 If you are not able to attend an onsite Biometric Screening,  your physician is required to complete a Biometric Data Reporting Form, in which you can obtain by clicking on the link below. 
Once the form is complete you can either fax it OR email it to Wellness Works!.

ONLINE HEALTH RISK ASSESSMENT
Once you complete your Biometric Screening, you must go online to your medical provider’s website to  complete your Online Health Risk Assessment (HRA).
