Financial Well-Being Incentive Program Guidelines
- The Financial Incentive Program is made up of REQUIRED and ANNUAL Activities. Required Activities can only be incentivized once. Annual Activities can only be incentivized once per calendar year. REQUIRED ACTIVITIES
- To be eligible for the Incentive Program complete each of the REQUIRED activities. By completing each of the activities in this section, you can earn up to $45.00.
- To earn additional incentives complete one or more ANNUAL activities. The more you complete the more you earn. Complete all three activities and earn up to $55.00.
- Employees may request financial well-being incentive funds not to exceed $200 per employee per calendar year.
- To be eligible for the Financial Well-Being Incentive Program, all REQUIRED Incentives must first be completed before an employee can be eligible for ANNUAL Incentives.
- All incentives will be delivered in the form of an Electronic Gift Card to the email address used to Sign Up for the Money Management Platform.

REQUIRED Activity & One Time Incentive
EARN A $25 VISA E-GIFT CARD - ENROLL & COMPLETE
ENROLL in the Money Management Platform powered by Enrich AND COMPLETE the Financial Wellness Checkup Questionnaire. You must complete both items to be eligible. Upon enrollment, complete the Incentive Claim Form provided at the bottom of the page.
REQUIRED Activities & One Time Incentives
EARN A $20 VISA E-GIFT CARD - TAKE YOUR MONEY PERSONALITY ASSESSMENT
Complete the Your Money Personality Assessment powered by Enrich in order to be eligible to receive your incentive. Upon receiving a copy of your assessment, complete the Financial Well-Being Incentive Claim Form provided at the bottom of the page to receive your E-Gift Card.
ANNUAL Activities & ANNUAL Incentives
EARN A $20 VISA GIFT CARD - DIGITAL FINANCIAL WELL-BEING EDUCATION COURSES/WEBINARS POWERED BY ENRICH
*Complete four (4) ENRICH digital courses (~20-30 minutes each) courses can be accessed through wealthworksCLE.enrich.org.
Upon completion of the coursework, complete the Financial Well-Being Incentives Claim Form provided at the bottom of the page to receive your E-Gift Card.
ANNUAL Activities & ANNUAL Incentives
EARN A $20 VISA E GIFT CARD - Financial Well-Being Coaching
Complete an Annual 1-1 Virtual Financial Coaching session with a Certified Financial Coach.

* Employees must provide documentation of completion. For a copy of the Financial Coaching Verification Form Click here. (Reminder: The Coach's signature is required on the form; present the form to the coach for signature).
** Once the session is completed, email the signed Verification Form to WellnessWorks and complete the Financial Well-Being Incentive Claim Form provided at the bottom of the page to receive your E-Gift Card.
ANNUAL Activities & ANNUAL Incentives

The Wealth Works Program will provide activities and challenges designed to expand your financial knowledge through gamification and interactive group challenges. Challenges are offered throughout the year. As they become available, employees will be notified.
The challenges are an opportunity to expand your financial knowledge though gamification and interactive group challenges. Challenges are offered throughout the year. As they become available, we will be sure to notify all employees.
*Employees must provide documentation of the completed Challenge or Activity. Forms will be available as part of the Wellness Challenge/Activity Rules.
(Examples include: registration forms, gaming forms, rosters, tracking forms, etc). Upon completion of the Financial Well-Being Challenge, complete the Financial Well-Being Incentive Claim Form at the bottom of this page to receive your E-Gift Card.

Employees may participate in one or more Challenges or Activities within a calendar year, but may not exceed the combine maximum value of $100.00/employee/year.
Certain prizes and awards such as gift cards may be considered taxable income. If taxable, the value of the prizes or awards may be subject to withholding and be included in the employee’s gross income and reported on Form W-2.
